Job Title: Care Coordinator
Location: Trowbridge Surgery, Hackney
Hours: 30 hours
Salary: £25,903.64 - Pro-rated
Responsible to: Practice Manager
Job Purpose
To provide care coordination and administrative support to patients, helping them access appropriate services and ensuring effective communication between patients, the practice and wider healthcare services.
The Care Coordinator will also support the wider practice team with telephone calls, reception duties and general administrative tasks when required.
Key Responsibilities
- Coordinate care and support for patients, particularly those requiring additional support.
- Contact patients by telephone to arrange appointments, follow-ups and appropriate services.
- Work with GPs, nurses, pharmacists and other members of the practice team to support patient care.
- Signpost patients to appropriate health, social care and community services.
- Maintain accurate and up-to-date patient records.
- Make and receive telephone calls and deal with patient queries appropriately.
- Provide reception cover, including welcoming patients and supporting the front desk when required.
- Assist with general administrative duties, including filing, scanning, referrals and processing documents.
- Support the practice with PCN and practice projects and initiatives.
- Maintain confidentiality and comply with practice policies, GDPR and information governance requirements.
- Work as part of the wider practice and PCN team to improve patient access and experience.
- Undertake any other reasonable duties required by the Practice Manager.

Essential:
- Good communication and interpersonal skills.
- Organised and able to manage workload effectively.
- Good IT and administrative skills.
- Ability to work independently and as part of a team.
- Professional, friendly and approachable manner.
- Understanding of confidentiality and handling sensitive patient information.
- Willingness to support reception, telephone and administrative duties as part of the role.
Desirable:
- Previous experience within a GP practice, NHS or healthcare setting.
- Experience of care coordination or working with patients requiring additional support.
- Knowledge of EMIS or other primary care systems.
- Understanding of PCN services and local community services.
