Are you a Diabetic Practitioner looking for an exciting new opportunity?
The Potteries are looking for a Practice Nurse or Pharmacist specialising in diabetes for 24 hours per week.
Experience in the management of diabetes either in General Practice or elsewhere such as a hospital or the community is essential to this role.
Experience in the initiation and management of patients on insulin is desired but not essential - training in this area can be arranged. Most of the work will be to provide care to our diabetic patients both through annual and interim reviews either as telephone consultations or face to face.
You would join a team of clinicians including a GP, pharmacists and health care assistants who work closely to provide care to the diabetic patients.
The health care assistants perform the initial physical check including foot examination and blood tests prior to the review. Although provision of diabetes care is crucial in this role, the successful candidate may also choose to be involved in providing other routine or urgent care to our patients such as general nursing, minor illness or chronic disease management.
The diabetes team meet weekly to discuss cases and learn from each other, and the GP is always available for advice.
We actively encourage shared learning and are a very supportive team. There are also regular treatment room and pharmacy team meetings.
Role Responsibility
Key duties will include:
- You will assist the registered manager with duties related to CQC outcomes, ensuring that compliance is maintained. As well as working collaboratively with the general practice team to meet the needs of the patients, ensuring practice policies and procedures are followed.
- Providing care for the presenting patient, treatment, chronic disease management and evaluation of care.
The Ideal Candidate

- UK registered nurse with significant post registration experience.
- Experience in cervical cytology and baby immunisations.
- Must have a good knowledge on quality outcomes framework.
