PCN Pharmacist

Job Introduction

We are looking for someone who is highly motivated and wants to develop themselves as Primary Care Pharmacist, prior experience is valued but not essential. You will receive training and development support led by our onsite lead GP and our training division. As a member of the practice team, you will be able to work closely with our clinicians to help improve patient care and make cost effective prescribing decisions. 

 

The post holder will be a qualified pharmacist, who acts within their professional boundaries as part of a multidisciplinary team consisting of GPs, ANPs, Physician Associates, Paramedics and First Contact Physios. 

Role Responsibility

Roles and responsibilities: 

  • Run daily telephone triage clinics and call in patients for assessment and management where clinically indicated. If prescriber, sign their own prescriptions. 
  • Provide practice based review clinics to those with LTCs or complex health needs including vulnerable people 
  • Review and triage incoming pathology results, contacting patients by phone where indicated 
  • Review and action incoming hospital correspondence, particularly with regards to medicine reconciliation or initiation, liaising with local pharmacies and CCG pharmacist (where indicated) to improve patient outcomes 
  • Lead practice repeat prescription services and deal with queries from reception staff and patients. If a prescriber, then sign the majority of repeat prescriptions 
  • To be participate in home visits to patient with acute and chronic issues 
  • To participate in immunisation initiatives including but not limited to influenza, shingles and pneumococcal disease 
  • To participate in practice smoking cessation services 
  • To be involved in practice administrative tasks – such as completing reports 
  • To provide remote clinical pharmacist access to other local AT Medics practices when required 
  • Liaise with community care colleagues to ensure medicines reconciliation 
  • Respond to Frailty requirements (as defined by BMA) and ensure patients’ needs are met appropriately, 
  • Provide clinical medicines advice to care homes and domiciliary care teams 
  • To follow up identified patient groups, targeting better patient adherence with treatment to improve outcomes 
  • Advising patients of the adverse side effect of medication and potential interaction with other medication/treatments 
  • Compliment and co-ordinate practice based public health campaigns with the local community pharmacy public health campaigns 
  • Present to Patient groups any changes to prescribing in line with NICE, best practice or in response to prescribing alerts 

The Ideal Candidate

To be considered for this exciting role you must have 

  • A can do attitude with a real desire to improve patient care and be self-motivated  
  • Ability to work well as part of a team  
  • Excellent communication skills both verbally and written and with a good team ethic
  • Willingness and ability to develop clinically 
  • GPhC registered Pharmacist
