Are you looking a Social Prescriber looking for an exciting new opportunity?

E16 Health are looking for an Social Prescriber for 37.5 hours per week.

Role Responsibility

Key duties will include:

  • The role will deliver a Social Prescribing Service to manage patient referrals from all primary care team members providing holistic assessments, co-designing a social prescription to improve health and well-being outcomes for individuals with a longer-term outcome of reducing the number of clinical /medical interventions required.
  • You will also be able to provide health coaching and more comprehensive support where appropriate for individual patients.
  • You will be able to work independently while escalating patients with complexity, who are in significant distress or who have safeguarding concerns.
  • The post holder will work collaboratively with other colleagues across the health, social care and the third sector to provide patient-centred, integrated care.
  • Develop and deliver organisational training modules that will as a minimum cover the role of social prescriber and health & wellbeing coaches, eligibility criteria and how to differentiate between low & high needs, patient experience and common themes/feedback from patients.

The Ideal Candidate

Interested? What you will need to be considered:

  • A health, social care, counselling or other relevant professional or academic qualification.
  • Excellent holistic assessment skills and experience of shared decision-making providing support to adults in a planned and structured way to improve health, recovery, and well-being outcomes.
  • Experience of training, supervising, motivating, and developing others to ensure a high-quality service that meets the needs and promotes positive outcomes for individual service users.
  • Proven skills in collating information and data on community resources and organising these in up-to-date and accessible formats for a range of different service users from various communities.
